The effective date reads October 2019 but it was just released today. You can begin using it now but the requirement is to have this in place by May 1st. Click here to be re-directed to the form.
Interested in what changed? It’s not extremely obvious at first glance…but if you read through the document you’ll notice:
- Section 1 includes minor changes, the most notable being additional countries in the Country of Issuance field
- There’s now some clarification on who can act as an authorized representative for an employer when completing this form
- Clarification regarding writing N/A in the identity document columns is no longer necessary.
- The new form also clarifies that the form’s List C documents doesn’t include a worker’s Employment Authorization Document (EAD), this is a List A document.
